Slips, trips and falls on the same level are the single most common cause of lost-time injury across almost every industry, rarely dramatic, but relentless, and occasionally serious or fatal when a fall is onto machinery, from a height, or into a hazard. They are also the most preventable, because the causes are known and mundane: wet or contaminated floors, trailing cables, uneven or damaged surfaces, poor lighting, and cluttered walkways. The primary control is housekeeping, backed by hazard recognition and the right footwear. EDIIIE’s Slip, Trip & Fall module, aligned to ISO 45001, trains workers to actively spot these hazards in their own environment, apply housekeeping standards, choose appropriate footwear, and report and correct hazards immediately rather than stepping around them. Every attempt is scored and certified, and the module works well as a universal component of site induction for every worker.
This module is designed around ISO 45001 and delivered as an immersive VR simulation with three progressive modes, Learn, Practice and Assessment, with progression gated on competency. Every trainee finishes with a scored result and a dated digital certificate recorded in the LMS.

The difficulty with slips and trips is not that the controls are complicated; it is that everybody underestimates them. A wet patch gets stepped around instead of cleaned; a cable gets left across a walkway “just for a minute.” Because the hazard is familiar, it becomes invisible. VR re-sensitises workers by putting them in a realistic environment and testing whether they actually spot and act on the hazards, clean the spill, reroute the cable, report the damaged floor, rather than walk past. It turns an abstract poster message into a scored behaviour, which is what shifts a housekeeping culture.
Unlike the high-hazard modules, slip-trip-fall applies to every worker on every site, office, warehouse, plant floor and visitor alike. That makes it the natural universal layer of a VR site induction, paired with the PPE and General Safety modules. One short, scored experience every person completes builds the baseline hazard-awareness habit that the specialist modules then build on. It is low-cost, high-coverage, and directly targets the biggest source of routine injury on most sites.

At Hindalco’s aluminium smelters, EDIIIE’s VR anode-change programme (“Pot Processing, The Hindalco Way”) trained operators for a process that runs beside molten aluminium above 900°C, toxic fluoride fumes and high electrical current.
